I hereby declare that I am fit to participate in competition, that there exist no health-related or medical reasons that would prevent me from race, and that I take part in the competition on my own responsibility and at my own risk.

I hereby declare that I have read the regulations of the competition, I fully accept it and undertake to comply with it.

I consent to the use of personal data contained in the registration form (in accordance with the conditions set out in Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data and repeal of Directive 95/46 / EC) by the Organizer to the extent and for the period necessary for the organization, conduct and promotion of the competition, selection of winners and collection of prizes (for processing). Data processing also includes the publication of the player's image, name and surname along with the name of the town where he resides, his age category, year of birth, gender, team / club name, result obtained during the competition - in any way in which it will be published or disseminated such information for the purposes of organizing the competition (including in the form of an SMS to the number of the competitor provided in the form).

Dear contestor, remember that your consent to the use of personal data and image publication is voluntary, but not consenting it prevents you from taking part in the competition, and us from achieving the goal contained in the Agreement (event regulations).
